This is a two-step process. Please complete this form in full. Once completed, hit Submit at the bottom and proceed to Step #2, the payment section of DEC’s Transcript Request page. If requesting multiple documents that are going to different locations, complete one request form per transcript. Requests are usually fulfilled within 5 days of receiving the request.
